No, "regressed eyeball" is not a disease. Here "regressed" refers to the placement of the eyeball fixture in a splay baffle for a slightly more concealed look. The 420W has a white on white finish that would fit in just about any décor style. You can order custom trim rings, too: the TRM6 line offers you a variety of finish options, and if you cut your opening too large, the oversize trim ring (#OT403) can cover up your mistake. Great product, great price, great service. Sometimes you just have to use the eyeballs for lighting with vaulted ceilings. These really update the look, they are a lot less noticable on the ceiling.
